On-Task / Off-Task Observation Form

Independent Seatwork Observation — Momentary Time Sampling
Observe the student during independent seatwork. At the start of each 15-second interval, glance at the student for ~2 seconds. If on-task (doing assigned work), click or tap the interval cell to mark it ✕. If off-task, leave it blank. Click a marked cell to unmark it. Use Table 1 below to calculate the rate of on-task behavior.

Behavior Intervals On-Task ÷ Total Intervals = Rate (decimal) × 100 = %
ON-TASK

Auto-calculates when both values are entered. Each 15-minute observation = 60 total intervals. Adjust total intervals if observation was shorter.
